Dr Johnson is such a good doctor(or surgeon or whatever) that she takes underqualified nurses under her tentacles and trains them how to properly apply incisions with or without any instruments.

She also keep some of her long time patients with her. She names them after which disease they have that she is curing them (or was it infecting, she gets those mixed up sometimes)
