Save setup, move a player, then load setup should make disappeared players reappear.

The missing player thing during setup happens when you accidentally drag two players to the same location quickly, and one of them gets squashed. If you do this during a kick-off result, most commonly Perfect Defense, but also Quick Snap or even High Kick, then you've hosed the game because you can't Load Setup to fix it. Solution: Take your time and be careful during those kickoff results.

My players were not dragged into the same position. I in fact saw that I had 11 players on the field. The game no longer recognized one of my players. Even though I could still move him. When I reloaded it, my player was gone. Just plain gawn.
On the other occasion when I had a blitz, my players were in the same position as I had placed them, but when I clicked on them the high lighted location was on an open square.My opponent saw them in different squares, and when I reloaded it was the set up the way that my opponent had seen them.
From recall, I dont think I dragged them into the same squares.

I've seen teleporting players too, I don't know exactly how that gets caused, but the symptom is as you describe - the coaches see the players on different squares. Probably the best action at that point is for both coaches to disconnect at the same time, and then both reload and see if they are back in synch.
